Requests & Registrations
Submit a ministry, event, or pastoral care request. Every submission is reviewed by our church office and pastoral leadership.
Please note: Weddings, baby dedications, church events, and facility-use requests for planned activities must be submitted at least 90 calendar days before the requested event date. Funeral and bereavement requests are exempt. Read the 90-day advance-request policy.

New Member Registration
Take the next step and become a member of Cornerstone Full Gospel Assembly. A pastor or member of our care team will follow up to welcome you and share the next steps.
- Baptism
Baptism Registration
Publicly confess Jesus Christ in the waters of baptism. Pastoral leadership will follow up to prepare with you. If you are requesting a specific baptism date tied to a scheduled ceremony, the 90-day advance-notice policy applies to that date.
- Wedding
Wedding Request
Request to hold your wedding ceremony at Cornerstone Full Gospel Assembly. All wedding requests are reviewed by pastoral leadership and must be submitted at least 90 days in advance to allow for premarital counseling and scheduling.
90-day advance - Baby Dedication
Baby Dedication Request
Present your child before the Lord in a baby dedication service. Requests are generally submitted at least 90 days in advance to allow time for pastoral review, scheduling, and preparation. Exceptions may be considered based on individual circumstances and pastoral approval. Every request is subject to pastoral review; submission does not automatically guarantee approval or a specific date. The church office will contact your family after reviewing the request.
- Homegoing Service
Funeral & Bereavement Request
Request pastoral care and coordination for a funeral, homegoing service, or bereavement need. Our pastoral team will follow up as soon as possible; the 90-day policy does not apply to time-sensitive bereavement requests.
- Event Planning
Church Event Planning Registration
Propose a new ministry event, program, or church-wide gathering. This form is for planning and approval of the event itself — for room/space reservations, please also submit a Facility Use Request. Event planning requests must be submitted at least 90 days in advance. Submission does not equal approval.

Reach out for pastoral care — for counsel, prayer over a personal matter, home visitation, hospital visitation, or spiritual guidance. Your request is treated with confidentiality by our pastoral team.
